If you were to visit medical care providers before reception of your Carte Vitale, please submit the documents you received from your health care provider (feuille de soins, etc.) to the specialized medical administrator following the steps below:

Please make copies of all documents received from the medical care providers, complete Section D of the medical claim form below and mail out all originals to M.A.I.

For all medical expenses incurred in France, please make sure to send the completed and signed original claim form (named “feuille de soins”).

All spectacles, medicines, laboratory tests (e.g., blood test), chiropractic care, etc. must be accompanied with a prescription from your physician, along with your receipt.

Please mail out your claims form to the address below within 12 months from the date of treatment.

Should you visit medical care providers after reception of Carte Vitale
If you were to consult a doctor after receiving your Carte Vitale, please hand your Carte Vitale to him/her. Billing and reimbursement will be done automatically through his/her card reader.
After you complete your application for mutuelle, you will be sent a certificate called "Tiers Payant", which you will need to present when you go to a medical facility (including hospitals, pharmacies, labs…). Please hold this card with you as you may be asked to show it.
Should you received a "Feuille de soin" from your doctor or a lab
Not all doctors and medical care providers can read Carte Vitale and process it automatically. Some may not have a card reader machine. In this situation you will be given a brown form by your medical care provider (feuille de soins). If you receive this document, you will need to go through a manual process of your reimbursement.
You must fill in this document your Sécurité sociale number, address, etc., sign in the bottom right corner and mail it to the nearest CPAM (Caisse Primaire d'Assurance Maladie).

[What is Carte vitale]
Carte vitale is a chip card which represents a proof of coverage by Sécurité sociale for medical expenses incurred in France.
You present this card when you go to a doctor, a hospital or a pharmacy. The IC chip will provide to the medical institution all info regarding your ID and administrative information.
The card is free but it takes a long time to be issued. It may take several months to receive your Carte Vitale after you applied for it.
